The Professionals Program of Elmhurst Memorial Healthcare offers the Comprehensive Assessment Program (CAP) for those clients requiring a thorough evaluation to determine fitness for duty, diagnostic clarification, and/or treatment needs. CAP is particularly suited to the assessment issues relevant to professionals, namely people with high responsibility, high accountability positions such as health care providers, attorneys, business executives, professors and teachers, religious leaders, airline pilots and flight attendants. The assessment, which occurs over a two to three day period, is multidisciplinary and multifaceted and includes the following components:
- HISTORY AND PHYSICAL EXAMINATION - performed by a physician board certified in internal medicine.
- PSYCHIATRIC EVALUATION - by a board certified psychiatrist.
- PSYCHOLOGICAL TESTING - including cognitive screening, personality inventory, and projectives when indicated, conducted by a licensed clinical psychologist.
- MARITAL AND FAMILY ASSESSMENT - performed by a trained marital and family therapist.
- SUBSTANCE USE EVALUATION - and chain of custody urine drug screen - conducted by a certified addiction counselor.
- VOCATIONAL ASSESSMENT - conducted by a licensed therapist experienced in evaluating role performance of professionals.
- COLLATERAL DATA COLLECTION AND REVIEW - from other relevant sources such as spouse, employer, EAP, hospital medical executive committee, etc.
All of these components are synthesized to produce a final detailed report. This includes a summary of findings and recommendations tailored to respond to the original question initiating the need for assessment.
FEE: The cost of Comprehensive Assessment is $3800 payable at the time of service.
